I/We do hereby confirm that I do not have any COVID-19 related symptoms like temperature more than 37.5 °C (99 °F) and/or flu, cold, cough , respiratory distress like symptoms.

I/We have not tested COVID 19 positive in last 2 months.

I/We do hereby agree to follow all the guidelines (Dos and Don’ts for the Guest) provided by the resort management and shall support them by adhering to the guidelines to prevent the possible spread of COVID-19 within your establishment.

I/We do hereby confirm that all information’s provided is correct and shall provide all the necessary supporting documents. Any information given found wrong & misleading – I/We agree that the local authorities have the right for penal action under the disaster management act.

I/We do hereby give my permission to your establishment authority to share my details including my mobile number with the authorities with required for the purpose of preventing possible spread of COVID-19 and its tracing.

I/We are not travelling from any containment zone or red zone area.

I/We undertake to strictly adhere to the protocol as prescribed by the health authority during the stay at the resort.

I/We undertake this stay is at my own risk of contracting any disease including COVID 19 and the resort & its management and staff will not be liable for any costs incurred or damages suffered upon either direct or indirect , as a result of or incidental to such illness and I release the resort from any liability of any kind arising from.

Do's and Don’ts for the Guest

Do not step out of the room unnecessarily. Wear a face mask whenever outside the room.

Guest are recommended to carry their own sanitizer and wet wipes for personal use.

Clothes should not be washed inside the room.

No visitors should be allowed in the rooms.

Always keep a safe physical distance of at least 2 m (6feet) while you are at the property.

Wash your hands frequently with the soaps/sanitizers provided.

Put all bio-degradable disposable plates/cups/bottles after use in the garbage bag.
